Nourish 52 Corporate Wellness Services Agreement

Taylord Success, LLC

This Corporate Wellness Services Agreement (“Agreement”) is entered into between Taylord Success, LLC (“Provider”), owner of the Nourish 52 Corporate Wellness Program, and the purchasing organization (“Company”).

By accepting this Agreement and submitting payment, Company agrees to the following terms:

  1. Program Overview

Nourish 52 is a corporate wellness benefit providing structured nourishment education, habit-building support, and guided resources for employees.

Each enrolled employee will receive individual access to the Nourish 52 platform, hosted securely via Kajabi, including unique login credentials.

Program features vary based on the selected tier:

Tier 1: Essential Nourishment

  • Platform access to the Nourish 52 curriculum
  • Self-guided nourishment education
  • No live calls, workshops, or individual coaching access

The Company will designate a Primary Company Representative and a Secondary Company Representative to serve as administrative contacts for program coordination, onboarding requests, and support communication.

If the Primary Company Representative becomes unavailable due to role change, leave, or separation from the Company, the Secondary Company Representative will assume responsibility. The Company agrees to notify Taylord Success promptly of any changes to designated representatives to ensure continuity of program support.

 

Tier 2: Guided Nourishment
Includes all Tier 1 features, plus:

  • Monthly group wellness calls
  • Quarterly wellness workshops
  • Q&A support during scheduled sessions

Tier 3: Whole Person Nourishment
Includes all Tier 1 and Tier 2 features, plus:

  • Enhanced onboarding support
  • Leadership-level implementation support
  • Ongoing engagement coordination with Company leadership or HR representative
  1. Contract Term and Monthly Billing Commitment

This Agreement is a 12-month contract, billed monthly.

Company agrees to:

  • Automatic monthly billing via the payment method provided
  • Maintain valid payment information
  • Fulfill the full 12-month contract term

Monthly billing is based on:

  • Number of enrolled employees
  • Selected tier level

Billing will continue automatically each month unless otherwise agreed in writing.

  1. Corporate Onboarding & Implementation Fee

A one-time Corporate Onboarding & Implementation Fee is charged at program initiation based on the total number of employees submitted during initial onboarding:

  • Up to 50 employees → $300
  • 51–150 employees → $750
  • 151–300 employees → $1250
  • 301–500 employees → $1,500

This fee covers:

  • Corporate account configuration
  • Secure platform implementation
  • Employee account creation and provisioning
  • Login credential setup
  • Initial onboarding coordination

This fee is non-refundable.

  1. Initial Onboarding Process and Timeline

After contract acceptance and initial payment:

Provider will send Company an onboarding spreadsheet.

Company must submit:

  • Employee first name
  • Employee last name
  • Employee email address
  • Assigned program tier

Once the completed spreadsheet is received, onboarding will be completed within up to fourteen (14) business days.

Employees will receive login access via email from Taylord Success through the Kajabi platform.  Email coming from: [email protected].

Company agrees to instruct employees to whitelist Taylord Success email communications.

Provider is not responsible for delays caused by incomplete or delayed onboarding submissions.

  1. Additional Employee Onboarding Fee

Employees added after the initial onboarding submission will incur a one-time onboarding fee of $7 per employee.

This fee covers:

  • Secure account creation
  • Login provisioning
  • Platform access setup
  • System integration and verification

Monthly per-employee tier billing will begin on the following billing cycle.

  1. Monthly Onboarding Window Policy (Operational Efficiency Clause)

To maintain operational efficiency and system integrity, Provider will process employee onboarding once per calendar month per Company.

Company agrees to:

  • Submit all new employee additions in a single consolidated onboarding request
  • Additional onboarding submissions will be processed during the next scheduled monthly onboarding cycle

Provider is not obligated to process onboarding requests outside the monthly onboarding window.

This policy ensures accurate system management, secure provisioning, and consistent onboarding quality.

  1. Billing Adjustments for Added Employees

When new employees are added:

Company will be sent a payment link to be paid before onboarding starts. Onboarding of additional employees will not begin until onboarding fees are paid:

  • The applicable $7 per employee onboarding fee, and
  • The applicable monthly per-employee program fee, beginning on the next billing cycle

Billing for additional employees are billed separately than initial setup employees.  

  1. Company Responsibilities

Company agrees to:

  • Provide accurate onboarding information
  • Submit onboarding requests in accordance with the monthly onboarding window
  • Designate a Company representative for program coordination
  • Ensure employees follow access instructions

For Tier 1, employees do not receive direct coaching or live support access.

  1. Payment Authorization

By accepting this Agreement, Company authorizes Taylord Success, LLC to automatically charge the payment method on file monthly.

Failure to maintain valid payment information may result in suspension of access. Provider reserves the right to suspend employee access for non‑payment or failed billing attempts until payment is restored.

  1. Intellectual Property Protection

Nourish 52 is proprietary intellectual property owned exclusively by Taylord Success, LLC.

Company and employees may not copy, distribute, reproduce, or share program materials outside enrolled participants without written permission.

  1. Platform Access

Access is:

  • Individual
  • Secure
  • Non-transferable

Login credentials may not be shared.

  1. No Medical Advice

Nourish 52 provides educational wellness support only and does not provide medical advice, diagnosis, or treatment.

  1. Agreement Acceptance

By accepting this Agreement and submitting payment, Company confirms that they:

  • Have read and understood this Agreement
  • Agree to the 12-month contract term
  • Agree to onboarding fees and monthly billing
  • Agree to the monthly onboarding processing schedule

This Agreement becomes effective upon payment submission.
